Physician-Dispensed Vaccines:

If a covered vaccine under Medicare Part D was dispensed and/or administered by your doctor, your doctor may collect your copay ONLY. The doctor will be reimbursed for the vaccine as well as the vaccine administration fee by submitting a claim to SilverScript using Form CMS-1500.
